What was the correct title of English Kings and Queen?

I have found different title wording and am not sure which one is corrent. I know that after Henry 8th English kings used the title Defender of the Faith and Supreme Head of the Church but i know that after ELizabeth the 1st the used Supreme Governor of the Church. I need to know what was the Proper and formal title of the English crown

The Current Monarch, Elizabeth II’s full title is as follows:

By the Grace of God, of the United Kingdom of Great Britain and Northern Ireland and of Her other Realms and Territories Queen, Head of the Commonwealth, Defender of the Faith

This varys for examble Her Majesty is also, Lord of Mann

the Sovereign is referred to as "His [Her] Most Gracious Majesty". In Acts of Parliament, the phrase "The King’s [Queen's] Most Excellent Majesty" is used in the enacting clause. In treaties and on British passports, the Sovereign is referred to as "His [Her] Britannic Majesty" as to differentiate from foreign sovereigns.
The Prime minister sits on the Privy Council and is Her Majesty’s Firtst minister.also leader of the house pf commons…<in reference to the girl ABOVE ME

    I always thought it was King/Queen…but Prime Minister? Wow, I really don’t know my British history….stupid American lol
